Baxter wins FDA De Novo for more expansive dialyzer

Renal care is Baxter’s largest business line and has become especially important during 2020 as acute kidney injury has proven to be a complication of COVID-19, raking in nearly $1.79 billion during the first six months of the year, up 5% year over year at constant currency. The need for dialysis therapies around the world is driving demand and FDA has issued a few emergency use authorizations for Baxter supplies.

Theranova, which given is considered a “medium cut-off” dialyzer as opposed to “high-flux,” may represent an important addition to that mix. The product has been commercially available in Europe and other parts of the world since 2016 and Baxter has already opened orders in the U.S. with an expected turnaround time of about a week.

The dialyzers are designed for use with conventional hemodialysis, which can occur in a clinic or at home — the latter setting has piqued the Trump administration’s interest as a way to reduce how much the government pays for costly in-center treatments. But the majority of clinical data available on Theranova is from in-center use, a spokesperson clarified.

The device was under review by FDA for almost a year. Baxter backed its FDA submission with findings from a randomized controlled trial of 172 hemodialysis patients, results of which were published in the Clinical Journal of the American Society of Nephrology this month. The trial, run across 21 U.S. sites between 2017 and 2018, compared outcomes in patients given the Theranova 400 dialyzer over 24 weeks to those given ELISIO-17H, a high-flux dialyzer from renal care product manufacturer Nipro. Patients receiving Theranova were found to have had superior removal of large middle molecules.

In early July, CMS proposed additional payment to providers for at-home dialysis treatment as part of changes to its ESRD PPS. Baxter, like fellow applicant and dialysis tech newcomer Outset Medical, is yet to be successful in obtaining the add-on payment. Read more

Medtronic artificial pancreas gets FDA nod for young kids

Regaining lost ground in the diabetes tech market is one of new Medtronic CEO Geoff Martha’s primary focuses. Expanding its product line to younger patients is one step.

Earlier this year, Medtronic published data from a clinical trial that assessed the use of its MiniMed 670G in children aged 2 to 6 years. Participants were in their target glucose range 64% of the time during the in-clinic portion of the trial and 65% of the time in the longer, pivotal period of the study.

Medtronic’s researchers concluded the data show the glycemic outcomes achieved in the in-clinic portion of the clinical study “were managed well and compared closely with those during the Auto Mode-enabled study phase of the pivotal trial.”

Now, Medtronic has used the data to bring a modified version of the 670G to market in children between the ages of 2 and 6 years old. The approval marks the first time such a device has been authorized for use in that age group. Medtronic and Tandem Diabetes Care previously only won approvals for their competing devices in children ages 6 or 7 years and older.

While the target demographic is new, the device itself is largely the same as the existing 670G. At one point, Medtronic looked set to make the 780G the focal point of its hybrid closed loop business in 2020. That plan changed earlier this year when the anticipated approval of the 780G was pushed back. In June, Medtronic CFO Karen Parkhill told investors the current plan is to “hopefully” launch the 780G in the U.S. this fiscal year, which ends in April 2021.

The 780G automatically adjusts basal insulin rates every five minutes and supports the selection of a lower glucose target than the 670G. While Medtronic has received a CE mark for the 780G and expects to launch the device in Europe in the fall, U.S. customers will have to wait longer. Read more

FDA proposes adding patient voice in device evaluation

FDA said it considers how patients feel, function and survive to be valid scientific evidence of safety and effectiveness that can be considered in the evaluation and surveillance of medical devices. The data should be considered voluntary and complementary to other outcomes evidence, but may be recommended in some standards and guidance, the agency said.

In clinical studies, a patient-reported outcome instrument could be used to measure the effects of a medical treatment or changes in a patient’s health. The information also could help determine whether a patient is eligible to be included in a study, and to measure primary or secondary safety and effectiveness endpoints, the guidance document suggests.

FDA encouraged manufacturers to seek feedback from the agency during the study design stage, before an investigational device exemption (IDE) is submitted or a pivotal trial is conducted, to determine the relevance and suitability of a proposed patient-reported outcome instrument to the benefit-risk assessment.

Further, outcomes important to patients should be measured and included in medical device submissions, FDA said. In addition to providing evidence of safety and effectiveness, the data could measure a device’s effect on patient well-being and other concepts that may influence healthcare providers’ decisions about possible treatments. Reliable patient data also could prove valuable in medical device labeling to communicate the effect of a treatment on patient symptoms, functioning or quality of life, FDA said.

The new guidance document comes nearly two years after the agency convened its Patient Engagement Advisory Committee to explore ways to incorporate the patient experience and priorities in device design, development and assessment. After the panel meeting, AdvaMed submitted comments to the agency that sought clarification on whether patient input would be sought from broader sources such as advocacy groups.

FDA said it encourages device makers to collaborate with patient organizations, medical groups and research institutions with expertise in patient-reported outcome instruments development. Read more

Clinical trial links hospital use of Dexcom G6 to improved glucose control

Hospitals typically perform point-of-care testing to manage the blood glucose levels of their diabetic patients. There are signs that approach is failing a significant minority of diabetic patients. Rates of hyperglycemia in non-critically ill hospitalized patients range from 38% to 46%. Hyperglycemia is linked to complications, mortality and longer stays in the hospital.

The Diabetes Care paper provides an indication of whether the use of CGM devices in hospitals could deliver benefits after the pandemic is over. Investigators randomized 110 adults with Type 2 diabetes to receive either Dexcom G6 or usual care during their stay in the hospital.

Patients in the G6 cohort spent 73% of the time in the 70 to 250 mg/dL range, compared to 64% of people in the standard of care group. Similarly, 33% of patients on standard of care experienced hyperglycemia, as defined by a blood glucose level in excess of 250 mg/dL. The rate in the G6 arm was 27%. The differences in rates of hyperglycemia and time in range were statistically significant.

However, CGM was statistically no better than standard of care at keeping patients in the tighter ranges of 70 to 180 mg/dL and 70 to 200 mg/dL. The hospital where the study took place aims to keep patients in the 100 to 180 mg/dL range and ADA guidelines list 140 to 180 mg/dL as the preferred target.

The failure of the Dexcom sensor to keep more patients in the tighter ranges therefore raises doubts about its value in achieving the targets set by diabetes experts. However, the authors of the Diabetes Care paper said the wider 70 to 250 mg/dL range “was a more realistic target” as patients enrolled in the study “were significantly dysglycemic at admission.”

The authors speculate the failure to improve time in the tighter range “occurred because our interventions were tailored toward giving extra rapid-acting insulin overnight for blood glucose 200 mg/ dL, a time when we otherwise would not be routinely using correction insulin.” Read more

Medical Alert Systems

Medical alert systems, also known as personal emergency response systems, offer a fast and easy way for the elderly, people with health issues, and those who live alone, to get help during an emergency, whether it be a medical issue, a fall, a fire, or any event that requires an immediate response. In a nutshell, these systems contain a help button that dials up an emergency response center and connects you to a live agent.

How Do You Choose a Medical Alert System?

The most basic systems use a landline and consist of a base unit and a portable help button you can wear around the house, but there are cellular options and GPS-based mobile solutions for people on the go, as well as options that automatically send an alarm when a fall is detected.

What Equipment Is Required for Medical Alert Systems?

The most basic medical alert systems use a landline telephone connection and consist of a base unit and a portable help button. They are incredibly easy to install; simply plug the unit into your phone jack using the included phone cord, and plug your phone into the secondary jack on the unit. Plug in the power cord and wait a few seconds while the system starts up, and when the LED indicator turns green, you are good to go.

It’s a good idea (and encouraged by providers) to run a test by pressing the help button on the base unit to make sure everything is working. When you are connected to an agent, simply tell him/her that you are conducting a test. They will verify your name and make sure everything is OK. Read More..
